Dental Screening Sign Up Form

This is a 1- minute look at your child’s teeth by a dental professional. This person is looking for fillings, cavities, and sealants. We use throw-away mirrors, tongue depressors, gloves, and hand disinfectant for these exams. Even if your child has had a dental exam recently, the information will help your child’s school and all children in Colorado. Your child will receive information about his or her dental condition so that you may follow up with your dentist or with a local dentist (we will provide a list of dentists) that accepts Medicaid, CHP+ or provides low cost dental care.
